Hospital to Home Care

A lack of proper post-hospital support is a leading cause of preventable hospital re-admissions. To ensure a smooth transition, we recommend planning post-surgery care at the same time the procedure is scheduled. Once you receive your discharge notice, we can finalize the dates for our caregivers to begin. 

Our caregiver will provide the individualized attention and support needed for recovery, such as following doctor's plan of care, helping with transfers, medication reminders, and meal preparation.
